Tender description

In partnership with Southport & Formby CCG and South Sefton CCG, NHS England North (Cheshire and Merseyside) is commissioning primary care medical services & management at Freshfield Surgery for a period of 3 years with an option of a further 2 years. NHS England North (Cheshire and Merseyside) has awarded a contract for Alternative Provider Medical Services for Freshfield Surgery. In partnership with Southport and Formby CCG & South Sefton CCG, NHSE (Cheshire and Merseyside) are looking to commission primary care medical services and management at Hightown Village Surgery and Freshfield Surgery for a period of 3 years with an option of a further 2 years.The delivery model should be innovative & in line with the principles of the General Practice Forward View. The procurement process will allow potential providers to bid for the 2 APMS contracts, respectively or collectively; Freshfield Village Surgery (Lot 1) & Hightown Village Surgery (Lot 2). The financial envelope is GMS rates, excluding Q&OF, NHSE DES and CCG commissioned services. Service requirements are to provide full time clinical services — Mon-Fri, in core hours from 8:00 am to 6:30 pm, so that patient needs are met in line with the GMS service specification, evaluation criteria and weighting, FMT and FMH, — Definition of full time clinical services–patients seen in core hours by appropriate clinicians.
