Tender description

The emphasis of the Reable Somerset service is to coordinate and join up all care and support, and use local information and resources for successful reablement. The outcome will be helping people in a different way, so that where possible, needs are reduced. This coordinated approach to care, health and community services allows for good interaction between professionals and people eligible for the service. It will identify local community based and voluntary resources to aid person centred planning and shared decision making. This will build greater resourcefulness within communities with a vision for self-care, prevention and active engagement. The Reable Somerset service will link with a range of other services being created that offer support for people in their own homes including: Ageing Well, Coast Falls project, Symphony projects in Taunton, South Somerset and Mendip areas. The Reable Somerset service is short term, and for a fixed period. It will focus on stabilising care plans, designing well-coordinated rotas and supporting smooth transfers to other appropriate services. The service has 2 key functions: 1. Coordination Role; 2.
