Tender description

The purpose of the Ofsted Registered (childcare register) Holiday Play Scheme Service is to provide access for children and young people with disabilities with social and leisure activities and to enable their parents and carers to have a break from their caring responsibilities. Short Breaks will help children and young people to engage, and be ready for, learning activities. Short breaks will also focus on providing the user with independent living skills. Parents and children must be involved in the design of the service. The provider will take responsibility for managing the process of profiling; risk assessing, support planning, delivery and monitoring/evaluation/review, once referrals are received. Each individual will have written care plans in relation to managing personal care, moving and handling, managing challenging behaviour and managing any medical needs (including allergies). Parental agreement will be sought in relation to the assessment, care plans, risk assessment and permission for off-site outings.
