Sefton Council is funding support plans of which the following service is required (note-this is a single package of care for one individual). YP is diagnosed Jacobsen Syndrome, severe learning disabilities and Epilepsy. YP has complex needs that require 24 hour care. A care package is provided to assist parents in caring for YP. A combination of agency and direct payment support is provided to enable 2:1 care requirements, to ensure the needs of other children in the family are not overshadowed, and to sustain positive family relationships Keywords: Learning Difficulties, Support, Care
