GB-Telford: Care Navigator Service

Tender description

Within community settings which include targeted enabling for vulnerable and socially isolated groups and admission avoidance signposting and support. The Care Navigator service will provide a supported signposting and short term support function to Service Users who are usually 65 and over. The Provider will receive identified Service Users who would benefit from support, typically provided outside of statutory health or social services. The aim is to support people to remain safe and independent at home, improve health, wellbeing & financial security and reduce use of statutory services.
