Hounslow Clinical Commissioning Group provision for Emerging Communities Services

The London Borough of Hounslow (LBH) has a diverse community with a variable and changing population. We are looking to work with an organisation to aid us in better understanding our newest and emerging communities and their healthcare needs. Hounslow Clinical Commissioning Group wishes to commission an organisation that can deliver an emerging communities service that will facilitate a strong link between new and emerging communities that will promote the dissemination of appropriate information and advice, promoting health and social care service provision, health and wellbeing and reduce isolation. The emerging communities service is designed to improve well-being and prevention with the intention to promote and produce accessible information and advice aimed at promoting a healthy and active lifestyle to a wider audience, support families affected by ill health or disability to live well and feel supported, preventing care and support needs from having a negative effect on family life and promote good practice in supporting people to make informed choices about their care and support and support safe use of services provided directly to individuals. Proposals should submitted via London Tenders Portal by no later than 12 noon on Friday 24 February 2017.
