Tender description
TEN ‘Transfer To Assess’ residential beds are required to complement the existing provision of ‘Transfer To Assess’ nursing beds, add capacity to alleviate seasonal pressures, and facilitate the safe and timely transfer of medically stable patients from an acute setting to a residential setting for further ongoing assessment of their health and social care needs. These 10 beds will be required for a period of 1 year and will be supported by the multidisciplinary Integrated discharge service.
