Tender description
NHS Wirral Clinical Commissioning Group is seeking expressions of interest from providers for the provision of a domiciliary care service to Wirral residents over the age of 18 years, with complex health needs who require end of life care. NHS Wirral CCG is seeking expressions of interest to provide a domiciliary care service to Wirral residents over the age of 18 years, with complex needs who require end of life care. This includes those who are eligible for fully funded NHS Continuing Health Care (CHC), or are in receipt of a joint funded package of care and have complex needs. The service must be able to receive and response to referrals 7 days a week during the hours of 7:30 am and 10:30 pm. The service is to be provided within a person’s own home. The majority of activity supports CHC fast track patients discharged from hospital in the last phase of life.
