Tender description

We are looking to establish a virtual ward model at KGH (KGHCARE@Home) with a baseline bed capacity of 20 with flexibility to increase to 28 beds, possibly to 33. Clinical pathways will include: — IV antibiotic provision, including osteomyelitis, cellulitis, joint infection, pneumonia, subacute bacterial endocarditis, urinary tract infection, — Non-weight-bearing patients, — Frail elderly – therapy, nursing, personal care, — Post-fall rehabilitation e.g. no fracture or fractures for medical management only, — Total knee joint replacements, — Wound care – VAC/larvae, — Bridging social care delays – Discharge to assess, delays for residential homes, delays for nursing homes, — Bridge package of care for stroke patients whilst community stroke team provides therapy, — Parenteral fluids and electrolyte replacement/monitoring, — Parentral Management of patients with hyperemesis, — Stage 1 acute kidney injury management. The service must offer 24hr healthcare professional support to patients. The development of recovery at home or virtual ward services offers a new method of managing this increases in demand and the flexibility within the fixed bed base to develop decant capacity over summer months to support the delivery of essential and planned maintenance. The service must offer 24 hour healthcare professional support to patient on the service and will have on-site clinicians to support the identification, consent, assessment and transfer of patients into the virtual ward. These staff will also support the ongoing clinical supervision and management of these patients in their own homes to ensure safe and effective care. Real time operational and quality performance metrics must be available to KGH in the management and utilisation of the service.
