Tender description

Wigan Council requires a preventative service that will offer support for vulnerable patients to go home safely following discharge from the Royal Albert Edward Infirmary (RAEI). The provider will work collaboratively with partners from Wigan Council, Wrightington Wigan and Leigh NHS Foundation Trust to target simple discharge and prevention of readmissions. This will improve patient flow and sustainability across the hospital and will reduce pressures on the NHS and support more people to be discharged from hospital when they are ready.
