"The aim of the Supported Discharge from Hospital Service is to provide people with support following an admission to Acute or Community Hospital or Accident and Emergency, so they can maximise their independence, self-manage their health and wellbeing and remain connected with their communities. Based at West Suffolk Hospital and Ipswich Hospital There is a Market Engagement session booked for Monday 11th December 2017 from 9.30am to 12.30pm at IP-CITY, 1 Bath Street, Ipswich, IP2 8SD.
