Tender description

THREE ‘Transfer To Assess’ nursing EMI beds (as specified in Appendix ONE) are required to complement the existing provision of ‘Transfer To Assess’ nursing beds, residential beds and residential EMI beds, add capacity to alleviate seasonal pressures, and facilitate the safe and timely transfer of medically stable patients from an acute setting to a nursing setting for further ongoing assessment of their health and social care needs. These THREE beds will be required for a period of no more than SIX MONTHS and will be supported by the multidisciplinary Integrated Discharge Service. The Integrated Discharge Service comprises: a) social work staff b) nursing staff c) therapy staff d) reablement staff e) hospital based staff f) administrative staff g) commissioned provider staff Keywords: Nursing, Nursing EMI, Care
