PP17149 - Admission Avoidance and Discharge to Assess service into a residential care home

This service specification outlines two discrete services that are being commissioned 1) Admission Avoidance Service, 2) Discharge to Assess service. The aim is to commission a small cohort of residential care home providers to deliver both services across the city. The requirements for 2 Admission Avoidance beds across the city and 4 Discharge 2 Assess beds across the city The purpose of the services are similar and are to offer a safe and responsive service to older or vulnerable people who attend the Accident and Emergency department at Royal Wolverhampton Trust (RWT) to avoid hospital admission when the person does not have a medical need to be admitted (Admission Avoidance) or to provide a safe environment for a person to be Discharged for Assessment in a care home because they cannot go home immediately (Discharge to Assess). There may be occasions when the Emergency Duty Team seek a residential placement for someone in the community to prevent admission through A&E.
