Tender description

The London Borough of Newham invites bids for Stroke Support Services for a fixed period of two years from 1st April 2018 to 31st march 2020. The Stroke support Service is a very specialist service that focuses on specified outcomes to achieve results. The Stroke Support Service will work with Community Neighbourhood Link Workers, Health and Social Care Navigators and Social Work Teams to ensure that they are aware of Service; and make appropriate referrals. The service will also need to plan and support challenges including but not exclusive, 1.Provide stroke survivors in Newham with support and advice after discharge from hospital 2.Provide a prevention service to all Newham residents, by raising awareness of stroke and risk factors 3.Work with GP practices across the Borough 4.Work with community pharmacies that provide AF and anticoagulation services 5.Provide awareness interventions to clients with pre-existing conditions. The evaluation for this service is based on 100% Quality (Technical) criteria only.
