Health and Wellbeing Fund 2017 - Social Prescribing

Tender description

The Department of Health along with its system partners Public Health England and NHS England are looking for bids from the voluntary, community and social enterprise sector for the VCSE Health and Wellbeing Fund 2017-18, in relation to social prescribing. As a result of identification of this opportunity, the Local Authority are asking potential applicants to come forward with a view to putting a partnership bid together. A key requirement would be to make use of existing resources within the Local Authority around social prescribing. For instance, the Council currently has a Local Area Coordination Team which identifies support available from the community that helps them with their assessed care needs. We would look for the Voluntary Sector partner to be the overall driver for the project and to coordinate with other community stakeholders.
