Tender description

The Care and Support at Home Framework is a joint procurement between Bournemouth Borough Council, the Borough of Poole and the CCG (Clinical Commissioning Group) known in this document as (‘the Commissioner’). Bournemouth Borough Council lead the Procurement. Care and Support at Home (known as domiciliary care and community home care) is a key option for people across Bournemouth and Poole, to help them to maintain independence and live at home for longer. This service aims to support vulnerable Service Users aged 18 years and above, to maintain their well-being and maximise their independence through the provision of high quality responsive care and support services to meet the outcomes identified in the Care and Support Plan/Health Outcome Plan, whether they are in receipt of either individual, or council managed budgets, or those receiving NHS Continuing Healthcare across Bournemouth and Poole. The start date of the contract is 1.11.2017. The Care and Support at Home service will be delivered between the hours of 6:00 to 23:00 365 days per year (24 hours for CHC) and is for all Service Users including older people, people with mental health conditions, people with physical disability including sensory loss or impairment, individuals with dementia, people in need of carers home based support or night care (waking and sleep in nights), live in care, end of life care, people leaving hospital and those living in extra care in Bournemouth. The Framework is for five years with the option to extend the contract annually for a further two years. Therefore, if all extensions were taken, the full length of the contract would be for 7 years. The start date of the contract is 1.11.2017. Extra Care Housing for older people 55+. 31 x 1-2 bedroom flats. 321 commissioned hours per week. Care and Support at Home (known as domiciliary care and community home care) is a key option for people across Bournemouth and Poole, to help them to maintain independence and live at home for longer. Extra Care Housing for people aged 60+ with dementia. 20 x 1-2 bedroom flats. 315 commissioned hours per week. Care and Support at Home (known as domiciliary care and community home care) is a key option for people across Bournemouth and Poole, to help them to maintain independence and live at home for longer.
