Integrated Wellness models aim to develop an integrated Health Improvement approach for adults; to help people build their capacity to maintain good health and be independent. Wellness is defined as a proactive, preventive approach that emphasises the whole person and which works to achieve optimum levels of physical, mental, social and emotional health. The Model will initially run for a period of five (5) years with the successful Provider, with the provision to extend the Model by up to a further five (5) years. The anticipated Service Commencement Date of the Programme is 1.10.2018.
