Tender description

SCC is continuing its ambition to enable health services and social care to work closer together. Our focus is very much on reducing NHS pressures and therefore overall system pressures and to do this we need to continue to innovate and provide more services in the community rather than in hospital based services. With the help of some recent pilots we are working towards recovery outside of acute settings and getting as many people back to their own home as soon as possible with the tools to retain or regain their independence. We would like to start the process of meeting with like-minded providers who wish to look at the best ways of working across systems and using our community social care experience to improve outcomes for people who are using health and social care. The future opportunities may include focussed multi agency reablement solutions but also looking at ways to prevent hospital admission rather than just focussing on delayed transfers of care after admission.
