Wakefield Council has developed a new model of provision for Housing Related Support, designed around: — Identifying the right services for people the first time. — Delivering long term solutions to a person's homelessness, with as few transitions as possible. — A Making Every Adult Matter (MEAM) approach — offering coordinated interventions that respond flexibly to meet assessed need. — A focus on harm reduction, recovery, resilience and independence; and — Promoting positive approaches to health and well-being. The model is comprised of 3 integrated pathways (detailed below) accessed through a single point of access. — Single Point of Access. — Assessment Pathway. — Access to Housing Pathway. — Main Provision. — Specialist Provision (this not included in scope of this procurement exercise). — Housing Sustainment Pathway. The Assessment Pathway will comprise of: — Preventing Rough Sleeping Service. — Assessment Centre. — Short term Supported Housing. Key aims will be: — To meet the needs of rough sleepers and prevent rough sleeping in Wakefield. — To create an opportunity to identify and understand a person's support need and identify the most appropriate support for them. — To support people to overcome barriers to gaining a tenancy. — To promote independent living skills and increase the service user's ability to manage and maintain a tenancy. — To support people to address needs around their mental health, substance misuse and/or offending behaviour. — To create opportunities for change and progression. — To promote positive approaches to health and well-being. The Access to Housing Pathway will provide housing related support to people who are at risk of homelessness and need support to both find and maintain suitable housing. The Pathway will support people to find and access accommodation and then provide outcome-focussed housing support in order to enable them to gain the skills to live independently. Key aims will be: — To support access to a range of housing options for people facing homelessness in the district; — To promote the use of the private rented sector as a viable housing option; — To support and enable people to plan for and achieve the best, realistic, housing solution for them; — To promote independent living skills in a way that maximises the service user's ability to manage and maintain their housing; — To promote positive approaches to health and well-being; — To promote opportunities for training, employment and volunteering; The Pathway will work with 175 single people and couples and 20 families (a total of 195 households at any 1 time). The Housing Sustainment Pathway will provide housing related support to people who are at risk of homelessness to enable them to keep and maintain their housing. Key aims will be: — To prevent homelessness. — To promote independent living skills in a way that maximises the service user's ability to manage and maintain their housing. — To increase service users' confidence in and ability to respond to issue with their accommodation. — To promote positive approaches to health and wellbeing. — To promote opportunities for training, employment and volunteering. The Pathway will work with 175 single people and couples;and 60 families (a total of 235 households) at any 1 time.
