Tender description

Support and Personal Care Service for Adults and Children. A flexible Agreement for Support and Personal Care Services that works to the Authority Commitment Statement; Right Help, Right Time to Promote Independence. This DPS is aligned to the national range of Government policies and initiatives for Support and Personal Care services and the Authority seeks Providers who are focused on a whole family approach. Support and Personal Care Services for Adults (18+). Non CQC registered Support and CQC registered Personal Care Services in Home and Community. Emergency CQC registered Personal Care Services in the Home and Community. The Authority are looking for Providers who will work to rapid response requirements to prevent admissions to Hospital, Residential and Nursing Homes where this is not entirely necessary and assist with facilitating Hospital Discharge in a timely and more efficient process. The Authority is working with the Clinical Commissioning Group to align work to the Better Care Fund and the Care Closer to Home programme. CQC registered Care and Support for Children and Young People (0-18). Providing better support for families to enable them to live well, also including improved support to looked after children, their carers and care leavers.
