Tender description
The Primary care Services (PCSS) will be provided from Kettering General Hospital NHS Foundation Trust site, at the from door of the Emergency Department (ED). The Key requirements for the PCSS are: — Provide the right care, by the most appropriate person, in the right setting ensuring consistent responses at the first point of contact. — Streaming non-urgent cases back to primary care. — To deliver unscheduled care within 4 hours transit time from registration to discharge, transfer or admission (includes onward referral to ED clinical team as clinic appropriate). The Primary Care Streaming Service (PCSS) will be provided from the Kettering General Hospital site located within the Emergency Department (ED). The key requirements for the PCSS are: — Provide the right care, by the most appropriate person, in the right setting ensuring consistent responses at the first point of contact. — Streaming non-urgent patients that attend the Emergency Department back to primary care. — Primary care GP working closely with their ED colleagues to collaboratively lead the provision of Primary Care Services at KGH. — Access for walk-in patients will be via the Clinical Streamer if appropriate providing a seamless pathway to any further assessment required including ED and referral (if necessary) to a hospital specialist. — Contributing to the Service development of Frail Elderly attendances at the ED from both the admission avoidance and discharge facilitation perspective. — Contributing to the development of ambulance patient access to the service. — Facilitation of unregistered patients to register with a GP practice. — Providing health promotion and self-management education to patients, relatives and carers. — Assuring facilitation of 1st Assessment completion within the designated 15 minutes, including NEWS and ECG where required. — Seeing and treating and discharging all patients within 4 hours of their arrival at the service. — Making all see and treat decisions within 60 minutes of the patient's arrival at the service. — Ensuring that necessary transfers occur within 60 minutes of arrival at the service. — Assuring completion of Medway for patients attending the service, with appropriate coding. Critically unwell and other seriously ill patients requiring emergency treatment will be referred directly to the Emergency Department at KGH without moving through the streaming service. The scope of the service is limited to urgent care and does not include a full range of services such as those which are provided by ED or GP practice. The service has scope for innovation and development for associated pathways including those to support the frail and elderly, ambulance patients and signposting patients to their own GP practice. As an integral part of this service, the Provider will work in collaboration with KGHFT. It is essential this service is flexible and will align with the Trust's Organisational Strategy in order to support the development of the overall 5 year Strategy which includes the integration of Primary Care and Secondary Care and supports programs of change such as Acute Frailty, Ambulatory Care, 111 and Out of Hours services. The agreed service protocols ensure that those patients in need of emergency care also receive that attention when required. The service will be expected to fully integrate with existing ED services to provide seamless care.
