Tender description

COPD Services as described in the contracts notice. NHS Halton CCG has a requirement for a Community COPD Service. Operating across Halton, the Community COPD Service will provide home (including residential and nursing homes) and community based care. The service must be able to deliver: — Patient centred care which allows patients with COPD to be more independent, taking more responsibility for their own care and quality of life. — Seamless care and support to patients and their carers through close working arrangements with social care and 3rd sector providers. — A ‘shift of care’ into the community, working primarily with general practice as part of a multi-disciplinary team designed around, and for, the patient and carer. — Referrals of appropriate patients to the Community Pulmonary Rehabilitation Service. The Community COPD Service will also be required to make a significant contribution to NHS Halton CCG's plans to reduce non-elective admissions, to effectively manage prescribing costs and to improve the management of patients with COPD in primary care. The contract will be awarded for an initial 5-year period with the potential to extend for a further 1 x 2-year period. The annual value of the contract is set at 750 000 GBP.
