Tender description
NHS Health checks for adults over 41 years. Design and deliver a primary care focussed NHS Health Checks programmes that maximises the reach and uptake of NHS Health Checks in key priority groups of the eligible GP registered population of Merton. As part of the Merton prevention framework and linked to the recently commissioned Healthy Lifestyle Service, the provider will be expected to maximise the reach and uptake of NHS Health Checks in priority groups. The NHS Health Checks service includes 4 key requirements: A. The re-design and safe transition and mobilisation of the programme by the successful provider. B. The delivery of a ‘Primary Care System(*)’ model for the delivery of NHS Health Checks. C. The development of an IT system to support the call and re-call of the eligible population. D. The design and delivery of quality standards, quality assurance and delivery of training.
