Tender description

The London Borough of Hounslow and Hounslow CCG are to establish a Pilot project for reablement care which will commence mid-November 2017 for up 18 months. The first 12 months of the Pilot is to measure the service delivery output to seek an efficient approach to reablement care at an increased capacity. The remaining 6 months of the Pilot is to re-design the service delivery model based on the evaluation of the Pilot performance, whilst at the same time tendering the service. The Council will be expanding an existing arrangement whereby up to three suppliers from the Council's Personal Care Framework will be selected to work on the Pilot. All information / data collected as part of the Pilot will be made available, upon request, after the 12th month. The value of the Pilot is c 800 000 GBP inclusive of additional council staffing resource.
