Tender description

The Council and Clinical Commissioning Group partners are looking to commission a fully integrated model of support for Carers. This will deliver a seamless service where there is a single phone number for all commissioned Carers services in Cheshire West and Chester borough. We wish to commission a lead organisation that will i) deliver a generic information and advice service; ii) manage the delivery of a variety of services delivered by a range of organisations; and iii) manage the carer break services commissioned by the Council and Clinical Commissioning Groups. There will be a single point of access where Carers have a triaged assessment and are offered a range of appropriate services dependant on their needs. We are currently conducting an options appraisal on the future of Carers assessments and support planning, should the preferred model be an external organisation conducting the assessment then the lead organisation will be expected to deliver this function as part of the triage function.
