Hertfordshire County Council is looking for supported living provider to take over an established service in Royston SG8 - Supported living service for 4 adults with learning disabilities based in Royston. - Delivering a commissioned service of 105 core hours with a shared sleep in provision for the 4 individuals. - Delivering the following additional 1 to 1 support for: o Individual A = 12 hours o Individual B = 60 hours o Individual D = 19.5 hours - Delivering outreach support to an additional individual in the community for 3 hours per week
