AQP Children's Continuing Care Service in Northamptonshire, Milton Keynes, Bedfordshire and Luton

NHS Nene CCG, NHS Corby CCG, NHS Milton Keynes CCG, NHS Bedfordshire CCG and NHS Luton CCG are seeking to establish a list of Approved Providers who can deliver high quality, outcome focussed packages of care in these communities to children and young people who meet the threshold for continuing care (CC) support in accordance with the service specification. The commissioners are looking for Providers who will deliver a service that provides high quality advice, information, support and intervention to ensure all children and young people with continuing care needs receive the support needed to meet their emotional, physical and medical health needs. The Provider shall deliver packages of care to support children and young people with continuing care needs to be cared for safely at home or within an appropriate community setting, which minimises hospital admissions. The Provider shall undertake a comprehensive but proportionate pre-assessment to ensure the needs of the child or young person can be met by the Provider.
