This event is being organised to engage with prospective providers and partners around the Humber Transforming Care Partnership support and care priorities. - To share good practice in delivering care and how the built environment can support this. - To consider the support required through multi-disciplinary teams to safely manage people in the community to avoid hospital admission. - Identify the career pathways, challenges and training needs for the workforce. - discuss the use of a joint charter to encourage a shared understanding of joint working arrangements.
