Tender description

Lot 1: Community Connectors: The service will engage the disengaged by setting up a network of new roles in the community to tackle the issue of social isolation and promote active inclusion which will improve and provide health benefits and lead to greater readiness for work. Lot 2: Non-Medical Therapeutic Recovery Service: This service will provide a pathway for participants to manage and improve their mental health/well-being and their readiness for employment via a personalised service that includes a blend of 1 to 1 assessments/review and facilitated group work offering a range of interventions including creative and physical activities. Lot: 1 Community Connectors: The service will engage the disengaged by setting up a network of new roles in the community to tackle the issue of social isolation and promote active inclusion which will improve and provide health benefits and lead to greater readiness for work. The Community Connectors will provide outreach and 1:1 support to individuals within the community to encourage greater access to existing services, social groups and activities within the community. In addition to this successful supplier will be expected to create structural enablers in the community such as volunteer Community Connector recruitment.
