Provision of Care Home Placement and Domiciliary Care Services

Tender description

University Hospital Southampton NHS Foundation Trust is looking to formally procure the Care Home Placement and Domiciliary Care Services for patients who live within and outside of Southampton City Council Boundary. The provider will be responsible for identifying a range of suitable care homes to provide the requisite of care for each patient's needs that are known to have capacity within the specified funding option. There will need to be identified a spread of care homes to ensure placement of patients as close as possible to their normal place of residence/preferred area of residence in the first instance, then escalate to appropriate University Hospital Southampton NHS Foundation Trust appointed lead if this is not achievable. The provider may need to take responsibility for arranging and facilitating domiciliary care packages as an alternative to care home packages if required.
