NHS Southport & Formby CCG and NHS South Sefton CCGs are working collaboratively to procure a Community Anticoagulation Therapy Service. The overall aim of the service is to provide a consultant-led, community-based anticoagulation service which will improve patient access to initiation of anticoagulation therapy and provide monitoring of warfarin across various clinic sites, and in patients' homes for housebound patients. The service will be delivered to all appropriate patients registered with GP Practices in South Sefton and Southport & Formby and will ensure equitable access and quality of service to the entire population group. The contract value is estimated to be between 2 500 000 and 3 250 000 GBP over the 5-year contract period. The overall aim of the service is to provide a consultant-led, community-based anticoagulation service which will improve patient access to initiation of anticoagulation therapy and provide monitoring of warfarin across various clinic sites, and in patients' homes for housebound patients. The service will be delivered to all appropriate patients registered with GP Practices in South Sefton and Southport & Formby and will ensure equitable access and quality of service to the entire population group. The service will be responsible for making specialist decisions regarding initiation of warfarin for patients referred into the service and provide ongoing monitoring and review of these patients. This will include regular reviews of TTR with recommended actions communicated to the patient's GP. In addition, the service will provide clinical expertise and input if changes to anticoagulant therapy is required (the service provider will manage any changes to therapy), implement bridging plans, and manage patients prescribed long term low molecular weight heparin (LMWH) therapy.
