Tender description

Kirklees Council is commissioning a Service which delivers and develops: — Dementia Friendly Communities — pre-diagnosis; — Information and Advice — post-diagnosis. The Dementia Information and Advice Service (DIAS) will work to raise awareness and understanding within communities in Kirklees pre diagnosis, signposting onto the diagnosis pathway where appropriate, and will offer all individuals being diagnosed with dementia access to a set of information and advice sessions. This Service will operate closely with South West Yorkshire Partnership NHS Foundation Trust who deliver the dementia diagnosis pathway across Kirklees. 1 Contract has been awarded, to deliver the full requirements of the Service Specification. The Contract is to commence on 16.10.2017 and will expire on 30.9.2019, with an option to extend for a further period, or for successive periods, of up to 3 years. The contract sum, for delivering the requirements of this service, is fixed at 155 000 GBP per annum, in accordance with Clause 5 of the Terms and Conditions of Contract.
