Market Warming Event for Community Nursing Homes Beds (incorporating Recovery and Physical Rehabilitation) for the Isle of Wight Population to be held 3rd October 2017. The services to be provided for people undertaking recovery and rehabilitation in a Nursing Home. Aiming to: • To provide an active part of an accessible and effective Recovery and Rehabilitation Service to ensure that adult island residents with physical disabilities receive interventions to enable them to achieve increased functional independence • To be able to support the recovery and "step up/"stepdown" phases for people who have recently been acutely ill • To reduce dependence on health and social care and enable self-management • To facilitate interdisciplinary collaboration and an integrated approach to assessment, treatment, care and discharge planning Interested Bidders must submit a message through the In-Tend portal through the messaging service for this project before 2nd October 2017, detailing the 2 attendees to the event and their organisation name.
