Tender description

The NHS Health Check programme is a mandatory public health service for the local authority. It is a systematic vascular risk assessment and management programme to help prevent various cardiovascular diseases (CVD) including heart disease, stroke, diabetes, dementia and kidney disease. The check is made up of three key components: risk assessment, risk awareness and risk management. The programme helps improve population health outcomes and quality of life by reducing the risk of vascular disease in adults attending for an NHS Health Check. It will help identify at an earlier stage, adults with previously undetected health conditions (specifically those that are listed above) not currently being managed in primary care and to signpost/refer to appropriate primary care and lifestyle services.
