High Intensity Users Service for Southern Derbyshire

NHS Arden & Greater East Midlands Commissioning Support Unit (NHS Arden & GEM CSU) on behalf of NHS Southern Derbyshire Clinical Commissioning Group (SDCCG) are inviting Expressions of Interest from suitably qualified providers capable of supporting a scheme focusing on High Intensity Users (HIU) of 999 / NHS111 / A&E, GP contacts and hospital admissions delivering for patients registered within Southern Derbyshire Clinical Commissioning Group. The Commissioners are assessing the markets interest, capabilities and capacity to support the evidence from Rightcare, which clearly illustrates potential for cost savings via the implementation of a scheme focusing on High Intensity Users (HIU) of 999 / NHS111 / A&E, GP contacts and hospital admissions. The aim of the proposed scheme is to reduce HIU activity and, free up front line resources to focus on more clients and reduce costs. The CCG's vision is to utilise a health coaching approach, targeting high users of services and supports the most vulnerable clients within the community to flourish. This HIU service was originally implemented in Blackpool (2013). It is the CCG's intension to develop a scheme lasting approximately 18 month, with a maximum financial envelope of £78,000.
