Tender description

The NHS Health Checks programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. Adults between the ages of 40 and 74, who have not already been diagnosed with 1 of these conditions, can be invited (once every 5 years) to have a check to assess their cardiovascular risk and be offered support and advice to help them reduce or manage that risk. The primary outcome of this programme is to reduce premature mortality and morbidity associated with cardiovascular disease (CVD). It should also make a considerable contribution to reducing the health inequalities related to CVD. The strategic aims are to: — Reduce the incidence of heart disease, stroke, type 2 diabetes and kidney disease; — Reduce premature mortality from CVD; — Early identification of pre-diabetes, hypertension, atrial fibrillation, high cholesterol and early chronic kidney disease (CKD) risk factors; — Narrow the health inequalities relating from CVD; — Raise awareness of the signs and symptoms of dementia. The Lead Provider will contribute by providing eligible Barnsley patients with a comprehensive health check programme which: — Identifies those most likely to be a higher CVD risk; — Provides them with a convenient and high quality health check including specified tests and measurements by appropriately qualified staff; — Ensures all individuals attending a health check gain a good understanding of their results and personal CVD risk; — Motivates these individuals to reduce their risk through self-driven changes to their lifestyle, attendance at appropriate services and/or engagement with ongoing primary care support; — And ensures appropriately coded health check results are embedded into individual patient records on GP systems to support ongoing GP follow up as appropriate. In order to do this effectively the Provider must secure good working relationships with GP practices to enable the necessary patient record searching for identifying the eligible population, targeting invitations, and delivery of some health checks in GP practice settings, as well as additional health checks in workplaces.
