Tender description

Bridgend County Borough Council's Social Services and Wellbeing Directorate wishes to recommission a specialist supported living service for up to four Individuals with a learning disability and complex challenging behaviour, including autistic spectrum conditions and is interested in bids from organisations with considerable experience and a proven track record in supporting people with a range of complex needs. The current contract is due to end on 31.3.2018 and it is an expectation of the recommissioned Service, that the Provider will work with Individuals, their Care Co-ordinators and relevant Multi-Disciplinary Teams, including Health Professionals to deliver a customised package of Care and Support for each Individual and to ensure that their health, wellbeing and safety in the community is met, as detailed in each Individuals' Support Plan and Positive Behaviour Support Plan.
