Tender description

North Staffordshire CCG and Stoke on Trent CCG are looking for expressions of interest for the tender of the memory support service. This service will provide support to patients and carers who are awaiting or have received a dementia diagnosis through the provision of a named care co-ordinator following the dementia well pathway. The service will need to be available/accessible to all residents of the two CCG's with innovative and individual support provided to both the patient and the carer. The service will help to prevent unnecessary admissions to hospital and assist with timely discharge when a patient has been admitted. Organisations wishing to express an interest and provide additional procurement-specific information should provide the following details by email to olena.florek@nhs.net and Elizabeth.johnson1@nhs.net by Friday 8th September 2017. The following information must be stated in the email: organisation name and address, contact name, telephone number, email address, website address, organisation type. Organisations should also confirm their registration on the Midlands and Lancashire CSU Bravo Portal https://mlcsu.bravosolution.co.uk. If not registered, organisations should follow the link and register on the portal.
