North Staffordshire & Stoke on Trent Dementia Advocacy Service

Tender description

North Staffordshire CCG and Stoke on Trent CCG are looking for expressions of interest for the tender of the dementia advocacy service. This service will provide advocacy support to patients with dementia and memory impairment to communicate, exercise and defend their rights and to fully participate in the decisions affecting their lives. This service will be part of the dementia well pathway and will be expected to work closely with the dementia memory support service. The service will need to be available/accessible to all residents of the two CCG's and will help to prevent unnecessary admissions to hospital and assist with timely discharge when a patient has been admitted.
