The service provides a mixed provision of support to people who are over the age of 60 and who need support to sustain independent lives in their own homes — supporting people to maintain accommodation and provide ongoing support to enable people to effectively manage and sustain their housing needs, with the aim of improving their ability to maintain their independence and avoid deterioration of health which results in admission to hospital or residential care.
