Tender description

Kent Community Health NHS Foundation Trust requires a Health Improvement Service IT system which will record all outcomes from clients wanting to access a service to support a behaviour change. This will include recording of an initial assessment, to booking an appointment and navigating the client to a service that is accessible to them, recording of further assessments once at the service entry point, recording follow up appointments and measuring long term behaviour change outputs.
