The contract is for the provision of primary care out of hours services in the county of Gloucestershire. The contract is for the provision of primary care out of hours services in the county of Gloucestershire. The aims and objectives of this service are to: — To be the point of contact for the Primary Care Out of Hours service. — To provide primary care out of hours urgent care for those patients who cannot reasonably wait until the in-hours primary care service is available again. — To provide a clear local patient care pathway which integrates and collaborates with the major health and social care providers in Gloucestershire. — To provide telephone and face to face clinical assessment. — To provide urgent medical cover out of hours to inpatients in community hospitals within Gloucestershire. — To support, where clinically appropriate, the admission of patients seen during the OOH period direct to Community Hospital Beds. — To work closely with the local provider of Extended GP Access (known as Choice Plus) to ensure alignment and integration across the OOH and Choice Plus Services and maximise productivity across both services. — To provide enhanced levels of clinical advice to patients and HCPs (including Paramedics).
