Tender description

County Durham and Darlington NHS Foundation Trust require a provider to supply GP services to the following Community Hospitals: Richardson Community Hospital, Sedgefield Community Hospital and Weardale Community Hospital. The awarded provider will supply a range of GP services. The contract will be for 36 months with the option to extend for a further 2 x 12 months. Richardson Community Hospital has 16 in-patient beds which provide step-up / step-down intermediate care, rehabilitation and palliative care admissions. Day hospital / care facilities provide elderly rehabilitation, elderly mentally ill, hospice respite and falls service. In addition numerous out-patient facilities are provided. Sedgefield Community Hospital has 16 in-patient beds which provide step-up / step-down intermediate care, rehabilitation, palliative care / end of life care, direct admissions for sub-acute infections. Day hospital / care facilities provide elderly rehabilitation, hospice respite and falls service. In addition numerous out-patient facilities are provided. Weardale Community Hospital has 16 in-patient beds which provide step-up / step-down intermediate care, rehabilitation and palliative care admissions. In addition there is a weekly day care hospice session which is overseen by the Community Palliative Care Consultant and weekly out-patient sessions for Physiotherapy and podiatry.
