Tender description

North Staffs and Stoke on Trent CCG's are looking to secure a service Provider to deliver structured education sessions to patients aged 18yrs+ who have been diagnosed with either Type 1 or Type 2 diabetes. The key aims of these sessions will be to empower and inform them about their condition, improve self-management, reduce crisis management & deterioration in health & reduce visits to GPs, Practice Nurses & missed appointments. Service Delivery is planned to commence in April 2018. In November 2015 the Staffordshire Health and Wellbeing Intelligence group produced a report on Diabetes Prevention. This report indicated that with improvements in awareness, early diagnosis and recording; and the current obesity trend; the prevalence of Diabetes across Stoke-on-Trent and Staffordshire will continue to increase and will be 8.7 percent by 2020 and 11.3 percent by 2030. The necessary lifestyle changes, the complexities of management and the side effects of therapy make education for people with diabetes a central part of diabetes management. As a result of this both North Staffs and Stoke on Trent CCG's are looking to secure a service Provider to deliver structured education sessions to patients aged 18yrs and above who have been diagnosed with either Type 1 or Type 2 diabetes. The key aims of these sessions will be to empower and inform them about their condition, improve self-management, reduce crisis management & deterioration in health & reduce visits to GPs, Practice Nurses & missed appointments. The main requirements for this service are to: Deliver standardised high quality, evidence based structured education that is consistent with NICE and all national/local standards and guidance on how programmes are delivered. Improve the quality of life for people newly diagnosed with diabetes enabling them to develop the necessary skills and knowledge to manage their condition. Have specific aims and learning objectives, supporting the person and their family members and carers in developing attitudes, beliefs, knowledge and skills to self-manage diabetes. Be delivered by trained educators who have an understanding of educational theory appropriate to the age and needs of the person, and who are trained and competent to deliver the principles and content of the programme. Empower adults with diabetes to manage their condition by supporting the development of self-management/self-care skills to reduce the risk of people with diabetes developing complications. The Provider will be expected to capture detailed information that can be used for audit purposes and that Commissioners can use to monitor services. The Provider will be expected to submit all information reasonably requested by both North Staffordshire and Stoke on Trent CCG to enable this monitoring to take place. A detailed service specification setting out the full requirements of the service will be provided as part of the tender documentation.
