Tender description
The aforementioned contracting authorities are working jointly as a Transforming Care Partnership (TCP) to deliver a programme to commission a small number of providers to work in partnership with us to develop and deliver bespoke and personalised care and support for individuals aged 16 years and over stepping down from secure services into the community. The TCP may also require providers to work with individuals at risk of admission. We are seeking high quality providers with proven expertise and experience of delivering personalised packages of support for people stepping down from secure services into the community and must have experience and knowledge in supporting individuals with learning disabilities and/or autistic spectrum conditions and in supporting individuals with forensic needs as appropriate. Services are likely to be for adults aged 18 years and over but the commissioner may consider services for young people aged 16 and 17 years stepping down from secure services, to prevent an in-patient admission as appropriate to needs and circumstances. The TCP is seeking to commission the least restrictive option for individuals with a preference for supported living or specialised housing with care where this can safely meet needs and promote the achievement of a range of outcomes. The TCP will commission registered residential care through these contracts where it is agreed that this arrangement is required to best meet the specific needs of an individual or group of individuals. As National Policy and new models of care evolve commissioners will work collaboratively with selected providers to deliver future arrangements that best meet the needs of our local population. This may require alliance and partnership models to be developed in future.
