The Provision of Proactive Primary Care and Pharmacy / Medicines Optimisation Support Service

Tender description

North Manchester CCG wishes to commission a pilot proactive Primary Care and Pharmacy/Medicines Optimisation Support Service for care home residents within the North Manchester locality; the service will provide care home residents with the appropriate level of support that they require. There are 33 registered care homes in North CCG with a registered population capacity of up to 1,096 residents (figures correct as at June 2016) It is anticipated that this will be a joint project between a lead Provider and other provider organisations to support the care gap for residents within the NMCCG care homes population. It is anticipated that the service will commence April 2017 involving the care homes registered within the new proposed Care Homes boundaries of neighbourhood three which has the largest care home capacity that equates to 8 homes. The service will then be rolled out to include all registered Care Homes (with the exclusion of identified learning disabilities homes until decision after 12 months review) so that by March 2018, all remaining care homes are included and integrated within the four North Neighbourhoods receiving the proposed service provision as part of One Team Place Based Care. This procurement will be run as a bespoke, single stage procurement procedure.
