Tender description
The NHS Health Check Service will be let to any qualified provider for an initial term of 3 years, with the option to extend for up to a further 2 years at the sole discretion of East Riding of Yorkshire Council. The purpose of the NHS Health Check Service is to assess eligible East Riding of Yorkshire residents aged between 40 and 74 years inclusive on their future risk of cardiovascular disease, and then provide tailored face-to-face feedback including Healthy Lifestyle advice and referral to further Public Health services where deemed appropriate. The Health Check Service will be paid at a fixed rate of £26.00 per Health Check completed for eligible residents and monitoring will be via the Outcomes for Health system.
