Tender description

Since 2012 EKHUFT, supported by the CCGs and KCC, have commissioned up to 80 ‘Health and Social Care Village beds’ within privately owned care homes to provide an enhanced and inclusive model of care which is able to meet the needs of patients on complex pathways including the NWB pathway, as well as patients requiring more enhanced levels of social care, whilst they undergo assessment and rehabilitation. This Tender is seeking to procure up to Eighty (80) beds across the whole of East Kent, which will allow patients to be cared for within an appropriate ‘non acute’ environment. These beds will predominantly support those patients requiring Pathway 3 (complex care needs, possible CHC eligibility or NWB pathway). A multidisciplinary team (MDT) comprising: nurses, therapists and social care manager will be provided in each location by EKHUFT and KCC and will include, and be supported by, a GP who will lead a weekly MDT meeting and provide medical input and care.
