Tender description

Contracted third party to work in liaison with the hospital discharge team and social care. The company would provide fully trained non clinical care coordinators to take referrals for patients who are medically fit and clinically stable for discharge, but who need to choose a care or residential home before being discharged. The patient and their family would be supported through the process of choosing and moving to a suitable care home with a dedicated and personalized care co-ordinator to accelerate the process. Procurement aim is to make a reduction in length of stay (LOS) for specific group of patients. This will result in financial savings due to the reduction in ‘lost’ bed days — where the patient waits in an acute bed without an acute care need. Procurement department of NNUH believes that there is no potential competitor in the market for this service.
