Tender description

The City Council's Supported Housing Pathway is designed to ensure that services make a positive contribution by improving the quality of life of vulnerable individuals through the delivery of personalised, preventative and early intervention services. The Young Persons Supported Housing Pathway provides a range of accommodation for looked-after children, care leavers and young people at risk of homelessness. The pathway enables the young person develop and gain the necessary skills to live independently including education, training and employment and an emphasis on good health outcomes. These services are located at the Cardinal Hume Centre and consist of 18 Emergency and Assessment Beds, 4 Training Flats and 14 High Support Bed-spaces. The Assessment and Emergency beds service provides direct access accommodation to facilitate the young person's safe return home, into private rented accommodation or move to a longer term supported environment and will also deliver a respite element. The Training Flats service is for care leavers who are more independent, in education training and employment and use support well. The High Support beds service provides young people, who may have a range of support needs, with intensive advice, information and support. The High Support beds service provides young people presenting with a range of support needs with intensive and structural advice, information, support. The service provides 16 high support bedspaces. The services will be provided at 25 Berwick Street, London, W1F 8FR. These services are located at Bravington Road, Shirland Road and Ashmore Road and consist of 4 High Support beds (at Bravington Road) and 11 Progress beds (6 at Ashmore Road and 5 at Shirland Road). The High Support beds service provides young people presenting with a range of support needs with intensive and structural advice, information, support. These services are located at Westbourne House and consist of 12 Progress bed-spaces. The progress bed spaces provide young people who may also have some low level concerns but possess a basic level of independent living skills, with advice, information support and guidance to help them become resilient and meet their aspirations. These services are located at 11-12 Gosfield Street and consist of 11 training flats. The training flats service will be required to test a young person's ability to live independently and is for care leavers who are more independent, in education training and employment and use support well. An additional support function consisting of a Referral Coordinator is required that will work across the supported accommodation services in the Young Persons Pathway.
